Appointment & Failure to Attend Policy

To ensure the smooth running of our practice, patients are requested to give as much notice as possible if they need to cancel/reschedule their appointments - a minimum of two working days. We understand that appointments may have to be cancelled or rearranged due to unforeseen circumstances. BUT failure to attend two or more NHS appointments will result in not being offered any further NHS appointments in line with the NHS regulations.

 

Failure to attend is defined as: 

​

  • Arriving late for an appointment (5 Minutes late or more). 

  • Not attending your appointment at all.

  • Not giving two working days’ notice.

 

We as an NHS practice have the right to ask you to find another dental practice IF you continue to miss appointments. We have a 2-year cycle, if an existing NHS patient does not attend our practice for a routine check-up within 2 years of their last visit, they will be automatically de-registered as an NHS patient at the practice. 

 

Private Patients

Patients are requested to give as much notice as possible if they need cancel/reschedule their Hygiene or Private Dental appointment- a minimum of two working days. We understand that appointments may have to be cancelled or rearranged due to unforeseen circumstances. 

 

  • Please note, no refund will be given for the Hygienist or Private Dental appointment whereby you have failed to attend or give the practice team sufficient notice. 

  • For short notice cancellations or failed appointments for Hygiene services a charge will be applicable, relative to the length of the appointment. No further appointments will be booked until this charge has been paid. 

 

If you fail to attend 2 appointments (including short notice cancellation) consecutively then we will contact you to establish a reason for the failed appointments and upon assessment, take the following actions accordingly:

  • Rearrange the appointment.

  • Be advised to find another dentist.

  • The Primary Care Trust will be informed.

 

As a patient at Ivy House Dental Practice, I have read and understood the above policy and agree to abide by this policy as I am fully responsible to keep my scheduled appointment.
